My electronic health records are secured, stored and managed within the United States.
Access to electronic records is restricted to Dr. Poth’s use only to execute job duties regarding treatment, payment, or clinic operations.
AI tools are NOT used for diagnosis or treatment decisions, if this changes, I will inform you.
Legal guardians may request access to a minor’s EHR upon request.
I do not collect, store or share patients’ credit score or voter registration status on my EHR platform.
The EHR system contains a dedicated field for biological sex at birth (male/female).
I can communicate with other healthcare providers regarding a patient’s metabolic health and diet for long-term chronic disease management.

To the extent that we have your substance use disorder patient records, subject to 42 CFR part 2, we will not share that information for investigations or legal proceedings against you without (1) your written consent or (2) a court order and a subpoena.
If we have your substance use disorder patient records, subject to 42 CFR part 2, we will give you clear and obvious notice in advance and a choice about whether to receive fundraising communications that use your Part 2 information.
